1. Site Purpose

The Site provides information and a way to submit inquiries related to dental care coordination. Unified Dental Solutions is not a dental provider, is not a medical or dental practice, and does not provide dental treatment, diagnosis, or medical advice.

2. Eligibility

You represent that you are at least 18 years old (or the age of majority in your jurisdiction) and capable of entering into these Terms.

3. Acceptable Use

You agree not to misuse the Site. This includes, without limitation:

  • Submitting false, misleading, unlawful, or harmful information.

4. Submissions

When you submit information through the Site, you agree that it is accurate to the best of your knowledge and that you have the right to provide it. You understand that submitting a request does not create a dentist‑patient relationship or any other professional relationship.

5. Communications & Consent

We may contact you by email or phone regarding your request. If you opt in, we may send SMS messages based on your selected consent category.

  • General Account Notifications (SMS) require a specific opt-in.
  • Marketing / Promotional (SMS) requires a separate opt-in.
  • Opt-out: Reply STOP to cancel SMS. Reply HELP for assistance.
  • Message & data rates may apply; message frequency varies.

6. Intellectual Property

The Site and its content (text, design, graphics, logos, and other materials) are owned by or licensed to Unified Dental Solutions and are protected by applicable laws. You may not copy, modify, distribute, or create derivative works from the Site content without permission.

7. Disclaimer
